Go to any local Thai immigration office and take with you 2 pp photos and cash (1900thb maybe).

Be aware that they may say that you don't require one as you have more than a single entry remaining (this happened to me). Just explain you understand and that you need it and the process should be painfree.

It's a single entry tourist visa

In the case of a single-entry, there should be no issues. Just keep in mind that the "re-entry permit" doesn't add any further days to the initial 60 days.
