On the TM 8 form outbound and inbound dates for a single permit can I leave blank

Was planning to stay until 2nd extension based on marriage was done next year, but have to return to UK earlier for a short period of time.

Yes or you can put any date as you are not held to them.

There is no such thing as an "exit permit"

For a re-entry permit there is no need to give any dates.
