3 month address notice for O-A multiple entry retirement visa

As of Jan 15, 2014,I have been here for the first two months on my O-A, multiple entry retirement visa. Last week, I got back from a one week vacation to Myanmar. Does my required 3 month address notice to the Thai immigration start from 2 months ago or from one week ago.?

Also, I note that the O-A visa rules say something about filling out a form when leaving the country. All I did was tell the immigration officer that I have an O-A visa. Was that sufficient?

The 90 days starts from last week.

There is no requirement to fill out any forms when leaving.

Although during the second year you will need a Re Entry Permit to keep your stay alive.

You report 90 days after re-entering Thailand, unless you leave Thailand again in the mean time. (So starting from 1 week ago).

No, you do not need to fill ut any form or anything to leave the country, only your departure card. But keep in mind the validity of your visa. It is valid for 1 year from date of issuance. Only during the validity of the visa itself (which you got from the Thai embassy) can you simply leave and re-enter Thailand.

For any return to Thailand after the visa itself has expired you will need to get a re-entry permit before leaving Thailand. That will keep your permission tos tay alive, otherwise you need to get a whole new visa abroad.
