Thanks for that info but that PDF form from DOE is the source of my question ,

what is it and where do I get it ? I just don't recall it from prior renewals.

It may not be required at all Labor ministry DOE offices. But that is where you would get the form.

If you go to page 8 of document you will find a form titled FORM OF EMPLOYMENT CERTIFICAT1ON.

More likely you would cite your Thai employer who is a 'Juristic Person'.
