The form asks "my address of record in the U.S.A. is" Do I put US address or my Thai address? This is for the US embassy in BKK. Also under occupation and monthly income. I'm a on social security disability. So I'm not working and nor can I because of Ed-visa. Should I put student with SSDI or none with SSDI? Then the amount. himmel Posted July 7, 2014 Share Posted July 7, 2014 Hi, im from the UK, (Retirement Extension) the form Illegibility to marry asks, Usual Address, i've lived at my g/f house for 3 years, so this would be the Address to put on the form, Yes, because my flat in the uk has been rented out for the last 8 years, and i have not returned the UK in that time. BlackJack Posted August 20, 2014 Share Posted August 20, 2014 I have 2 forms from the US Embassy can download online one is a Divorce and the other is a Marriage Affidavit The are statements and not actual proof of anything other than that person filled out the form and signed it in the presence of an Embassy authorised to sign off on it. The Divorce Affidavit states that the applicant was previously married and the marriage was terminated by Divorce. The Marriage Affidavit states that the applicant is "single divorced or widowed" and "qualified and eligible to marry" So my question is which one do I need to satisfy the Thai Authorities? thanks Link to comment Share on other sites More sharing options...
