Hypoxia, particularly the entity 'diffusion hypoxia', can occur with the administration of inadequate amounts of oxygen during or immediately after a N2O anaesthetic. N2O will diffuse into air-containing cavities within the body faster than nitrogen diffuses out. This results in a temporary increase in either the pressure and/or volume of the cavity depending upon the distensibility of its walls. The magnitude of the effect is proportional to the blood supply of the cavity, the concentration of N2O inhaled and the length of time the patient is exposed to N2O. Significant morbidity or even death can result from this phenomenon. A property unique to N2O is its ability to oxidise and inactivate the vitamin B12 components of certain enzymes in both animals and man. One such enzyme, methionine synthetase is essential for normal DNA production. Animal and human studies have demonstrated that the haematological, immune, neurological and reproductive systems are each affected. These adverse effects of N2O can occur after both acute (surgical) or long term (occupational) exposure to the gas. Because of its effects on the pressure and volume characteristics of air-containing spaces, N2O should not be used for patients with bowel obstruction, pneumothorax, middle ear and sinus disease, and following cerebral air-contrast studies. Many anaesthesiologists feel that use of N2O should be restricted during the first two trimesters of pregnancy because of its effects on DNA production and the experimental and epidemiological evidence that N2O causes undesirable reproductive outcomes. Since N2O affects white blood cell production and function, it has been recommended that N2O not be administered to immunosuppressed patients or to patients requiring multiple general anaesthetics. Many anaesthesiologists believe that the potential dangers of N2O are so great that it should no longer be used at all for routine clinical anaesthesia. However, the continued use of N2O remains a controversial topic since, at present, a suitable substitute gas is not available. Then read this if you have the energy: Short-term dangers of nitrous oxideNitrous oxide can produce acute adverse effects in some users, and some of its dissociative effects may represent dangers to the user. Possible adverse effects of nitrous oxide include: nausea (especially when used repeatedly, after a meal, or in combination with alcohol) which can lead to vomiting (can be dangerous to the user, because he/she can fall unconscious on nitrous oxide and choke on his/her vomit) and headaches, during or after the experience. Because nitrous oxide can cause loss of motor control, inhaling it when standing can be dangerous. Nitrous oxide should not be used in any situation in which sudden unconsciousness may be dangerous (standing, driving, operating machinery, etc.). Users should be in a comfortable and safe setting to avoid accidentally inuring themselves. Serious injury and death has resulted from nitrous oxide being inhaled while driving or standing in front of an open window. Also, in a scientific study, it was shown that 24 hours of exposure to nitrous oxide caused interferences with DNA synthesis.When nitrous oxide is used heavily and over an extended period of time, vitamin B12 depletion will probably become a major problem, as it can cause brain and nerve damage. Physical symptoms of vitamin B12 deficiency include: tingling sensation in extremities, paresthesias (no sense of touch and pain), difficulties with motor control (can lead to inability to walk), etc. If a frequent user of nitrous oxide experiences one or more of the above symptoms, it is vital to cease the use immediately and consume healthy amounts of vitamin B12 and other vitamins (especially folic acid). If symptoms persist or get worse, a physician has to be consulted. There are some reported cases of chronic nitrous oxide use resulting in the symptoms mentioned above. However, virtually all of the patients used nitrous oxide excessively, up to 400 whippets each week, over the period of several months. If nitrous oxide is used frequently (although this is not recommended), consuming supplements containing vitamin B12 and folic acid may reduce the long-term risks of chronic nitrous oxide use.Heavy use of nitrous oxide can result in lower amounts of lymphocytes of the blood (results in a weakened immune system), reproductive disturbances and hyperhomocysteinemia (a risk factor in vascular disease).As the effects caused by nitrous oxide are pleasant and very short-lasting, many users desire another dose. Sometimes, this continues until no more nitrous oxide is available to the user. This compulsive use of nitrous oxide can lead to psychological addiction in some users, which can lead to excessive use (dangerous for the reasons stated above) and deterioration of personality.Prolonged exposure to nitrous oxide has been shown to cause Olney’s Lesions, although the alveolization of certain parts of the brain does not result in further damage as happens with most other dissociative drugs. High doses of nitrous oxide have been proven to be teratogenic in rodents, and also other conditions caused by chronic use of nitrous oxide (vitamin B12 depletion, hyperhomocysteinemia) can result in a malformed child if the user is pregnant.Because nitrous oxide tends to diffuse into air-filled spaces in the body (intestines, middle ear, etc.), it may be dangerous when used by persons with middle ear disease, bowel obstruction, pneumothorax, etc. In the case of nitrous oxide in the intestines, it will probably only result in mild gastrointestinal discomfort. Nitrous oxide in the middle ear may cause temporary or permanent hearing loss, which is why nitrous oxide should not be used when one has an ear infection or any other disease in this region. Pneumothorax (collapsed lung, accumulation of air in the pleural cavity) is a medical emergency. Nitrous oxide should not be used for analgesia in such cases, but medical personnel will (hopefully) be aware of the dangers of administering nitrous oxide in these circumstances. The operator said western tourists always asked for the laughing gas, prompting many shops, especially those owned by farang, to provide it. Fun police are definitely out in force on this one. As others have said the buckets of turps they have been drinking was probably more of a contributing factor (and far more detrimental to their health). You've got 3 young girls in their prime "partying" years having a bit of a tear up. Are the being a little stupid and irresponsible... probably. Are they bringing Phi Phi island into disrepute IMHO hardly... are the evil farang the ones are dispensing the majority of the nitrous... not bloody likely. Now most of you old farts will be tsk tsking and calling for their heads but these islands have been promoted as party destinations since Adam was a boy. If the junta wants to stop this sort of behaviour then they need to start with the closing down of every bar in Thailand. Maybe start with the ones where mushrooms and ganja and extasy are so readily available hmmmm or maybe some people would be missing out on too many brown envelopes This old fart was on Phi Phi in the early seventies ( not an old fart then) and it was certainly not promoted as a party destination- it was stunning- just half a dozen people on the beaches . No drugs, no bars , basic hotels. Am very lucky to have travelled the world before places were totally ruined by the bar / drug culture

What interest me more is that I've seen here in Thailand many times farang, all-female groups drinking in night clubs, parting on beach ... July-September, Christmas, beginning of the year. Obviously drunken, *high* and almost naked. Young individuals, 20 years old, students. Who pay for that, their parents, or do they have some extra money from last summer job? Guys and nightlife in Thailand, ok ... but girls? I mean ... What's gonna be next? What do you mean, that girls shouldn't have fun? What's gonna be next; more girls, I should imagine; they're not hurting other people or forcing others to do as they do - good luck to them - get over it - sometimes girls just wanna have fun! I've always considered women to be a better part of society, responsibility, ethics and so on ... BTW, if this is for you fun, normal activity ... Then there's something wrong with you. With all the respect. What would you say if it's your daughter, She just have fun? Come on ...
