Patong Immigration - Can you do notification of residence? (not 90 day report)

I need to go to Phuket Town immigration soon for my ED Visa extension, but my school are saying I first need to make sure my passport has the "notification of residence" in it since I changed address since last went to immigration.

I know the owner of the house/condo etc is supposed to register you, but mine doesn't really know what to do and if I have the signed documents needed I can go do it myself.

So I know I can do it at Phuket Town immigration (did that last year), but since I'm in Patong the question is, can I do it at the immigration office here as well? I know they usually only deal with tourist/short term visas and such, but since they do 90 day reporting I'm wondering if their system can also register my residence details.

You most go to Phuket Town Immigration and you need.

I did this for 2 weeks ago and then I had to have all this ready.

1. Copy off the rent contract.

2. Copy of the Land lord ID card.

3. Copy of the House book. ( Gray book I think)

4. copy of your Passport.

5. Creating a drawing of streets where you live. (Not Google copy map)

6. Picture off the house whit house nr.

I don't wanna create confusion but I've been to the Phuket Immigration Office in town one week ago and the form wasn't quite like the same as the TM30 you can download from that page, i.e. http://immigration.go.th/nov2004/download/pdf/tm30.pdf ... the receipt at the bottom is basically the same (the one that remains in one's passport) but the part to fill out is different.

I have read that they will accept a TM30 at the registration desk. But you have get past the check in desk to get there I understand.

No intention to confuse the issue, Phuket is the only office that uses it's own form in that respect (as far as I'm aware).

Very useful for Joe to post the pdf for their form.

It should be pinned in the Phuket forum for download by Phuket expats.
