Hello,

I will be going to get my current visa put into my new passport this week at Chang Wattana. Do I need to fill out any forms? (Like to have any relevant forms all filled out before I get there)

Do I only need the letter from my embassy?

You need letter from embassy and copies of old passport/visa/arrival/extensions and new passport as well as filling out a short form (which you can do at office in a minute or two.

It has no official number - but is a standard form here in Bangkok. Copy stamps to new passport form

OP - if you literally mean a visa, please note that this cannot be transferred from your old to new passports by Immigration (or anyone else for that matter), meaning that you will need to carry both passports around with you until the visa has expired.

On the other hand, if you are referring to an extension of stay stamp, then the procedure you'll need to follow is as advised above.
