Pilot error caused offshore helicopter crash, according to the UK Air Accidents Investigation Branch in its final report on the fatal incident.


The report released Tuesday lays out the events leading up to the fatal crash on 23rd August 2013; and identifies the cause as combined pilot error, recommending changes to Standard Operating Procedures (SOPs) for UK offshore helicopter operators.
