I am going to my first 90 days report and understand I first need to register my address,what are the requirements for the address registration ?

It depends upon which office you report to.

You will need a rental agreement with copies of the owners house book and ID card at many offices.

Some will also want completed TM30 form from your landlord or the owner.

At one time Phuket immigration was a wanting a special form of their own for the registration instead of the TM30 form.

You will need proof of residence such as the rental agreement I mentioned.

If coming from a short holiday in cambodia do i have to report to immigration as i go back to mij own house

That depends upon the office you report to.

Some say you don't need to do a TM30 unless you move and others after you leave and re-enter the country.
