Ok, maybe a couple of stupid questions, but I'd rather get it right than do it over.

 

Please confirm:  Visa Type:  Non Immigrant "O"

Arrived by - Should be my most recent entry into the Kingdom, i.e. where I got my current departure card, not where I arrived from when I got my original visa, yes?

Address in Thailand:  The complete address, as it appears on my housing lease (which is my proof of residency document)
