This Thai visual expression means: stimulation of the female genitals using the tongue or lips. So if the OP girl is one of your girlfriends, I would not recommend taking her home and introducing her to mother.

I hate when people, male or female, Thai or farang, make that gesture.  In my experience, when guys do it, they're most likely total douchebag <deleted>s.  When women do it, they're, shall we say, usually "all talk and no action," haha.

Probably depends on the person. Some would do to it with a sexual implication, many would have no idea what the general meaning as and have just seen others do it. Like the peace sign. 

Many people from different Asian countries have told me they just feel insecure about what to do with their arms when having a photo taken, so they just do stupid poses.
