Anyone out there have any recent experience with getting a Thai tourist visa in either Kathmandu or Dhaka (for an Italian passport holder)?

Not sure about Kathmandu since they have this on their website. http://www.thaiembnepal.org.np/?do=services&index=visa&lang=en

To prevent the security issue that may occur, the Embassy has reserved full rights to refuse the visa for the foreigner who are non-residence in Nepal and travel into Nepal for the tourist purpose. The foreigners who have visited Thailand before may be exemption for this standard.

You should contact them to confirm whether they will do one for you or not.

I think Dhaka will do one for you. You would need to contact them to confirm.
