Depends on the type of doctor. The Thai doctor that did my knee op studied at Harvard and worked at the Police hospital and Bangkok Hospital. I presume the latter paid more than the former. I presumed he was from a wealthy background because of him going to Harvard. Sometimes, there is no conspiracy.

It's all down to the individual car. Selling a reliable car to buy one almost 20 years old and free money up for investment is interesting. I would say that maybe keep back 250k for repairs. To hold back 0 for repairs and expect reliability similar to your existing car - wouldn't be very savvy.
