I think I am getting you here. The "Yellow Cog Shop" is providing a service by going to the Land Transport Office for you. That's why you are paying everything together, they are a one stop shop/facilitator.

You may want to go on the medical forum and consult Sheryl. She has given some excellent advice in the past on surgeons etc. I followed her advice and had some great work done on my knees and back by a couple of surgeons in Bumrungrad. Full mobility and back running, cycling and swimming.
