You could have contacted Bkk Bank and asked 7-10 days earlier. See my suggestion.

Go onto the Bkk Bank APP, at the bottom right is More Services, Request Account Statement, Account, Time Period, Language, Submit. The Statement will be emailed to your registered account with an Attachment which is locked until you put your DoB in, ddmmyyyy, and it should take less than an hour. Print it out, and show the IO that it came directly from Bkk Bank. He may accept it.

However, forename is not flagged as such, even though it is required

It can be confusing when filling in Thai Immigration papers, when for example, the boxes to fill in dates have the day at the end of one line and then month and year at the start of the next. WHY they cannot have dates to be put as the conventional DD/MM/YYYY I cannot understand.

Done my last 6 90 day reports online to Phitsanulok Immigration, even the first after a trip abroad. All accepted and replied to within a day or two.

Why take the machine with you. Get a free app and put the data in your phone every time you take it, in my case every morning. The app can work out averages, highs & lows etc.

And, in my hospital, give different readings on different machines. Get an OMRON from lazada for less than Bht 2000, and D I Y.
