If you want a full year extension, you must renew your passport. If use old, you will only get an extension until the last day of your passport validity.

As you stated yourself, your best option is to return to UK as quick as possible. As other have stated, you might be lucky and get your new passport there, before your permission to stay/extension ends. Just do not forget to apply and pay for a single re-entry permit before you leave.
