I was given that form together with some other forms in order to prepare my next application for extension of stay (based on retirement). allane Posted January 25, 2018 Share Posted January 25, 2018 Have you been asked to submit a map showing the location of your house ? The upper part is for your name, address and phone number. The two choices at the bottom (of which you presumably will circle the left one) are "house map" and "work map" Link to comment Share on other sites More sharing options...
