Date to put on reentry permit application?

My Non-O immigration retirement visa original validity ended last June.  Before it ended, I went out and came back in, and now have permission to stay until June 2019.

 

Maybe a stupid question on my part, but on the application for multiple reentry permit, where it says "I arrived in Thailand on", do I put the date I first arrived on my visa (last year), or the date of my last entry (this past June)?
