Builder And Structural Engineer Recommendation ?

After several horendous experiences, I want to ensure my next home extension project doesn't suffer similar problems, mainly

1. flawed foundation design

2. poorly executed foundation construction

3. general construction problems caused in part by lack of supervision.

Can anyone recommend a competent building company, and an independent structural engineer, that will do small domestic projects ?
