As to the question of medical insurance that is all well and good if you can get it. Due to retirees ages some just can not get it. How many long stay retirees would be willing to pay a fee to use the government hospitals. Similar to those working and paying into the Thai SS system. Then that money would (Hopefully) go directly to the government for healthcare and not to feather the nest of insurance companies.  Retirees I have spoken to would be more than happy to pay the 400+ Baht /month that employed expats pay to get a health card to use in government hospitals, even willing to pay more say 1,000 to 1,250 Baht per month or 12,000 to 15,000 per year. Anyone want to start a poll.

Good idea. It is the damned for profit private hospitals that have caused the mess we are in     No good saying get insurance ,for the aged it will be sky high.

Still for the decades i have not had health insurance I must have saved a fortune, an absolute fortune, well enough to pay my way through the govt. hospitals
