Wondering if you can guide me as I fill out the TM.8 (re-entry permit form)? 

 

I understand all fields except the last part: 

 

I arrived in Thailand on ......... 

Is this the date on my departure card? (the last time I entered Thailand?)

 

and have been permitted to stay up to ........

Is the the expiration date on my current extension of stay stamp?

Arrived in Thailand is your most recent entry to the country.

It is the date your extension ends which is your permitted to stay date.

John, if your flying out from swampy airport they can fill in form for you and provide photo. All for 200baht. Might save you stress downloading tm8 form etc. 

I always do my own but is an option at bkk airport.
