The form is asking for 2 addresses. I assume the second address is the new address but what address is needed for the first section?

The first address is for the person completing the report.

The 2nd address is for the address being reported.

Both can be the same if you are reporting yourself.

The form is pretty self explanatory, the 1st address is the address of the person completing the form (owner, housemaster, possessor). The second address is the property where the report is for.

If one is registering as the Possessor, what documents besides copy of passport, need to be uploaded?

I want to register at CW, but the field "Register at" is greyed out. 

So if someone on a tourist visa goes to do a 30 day  extension at CW can he/she just pay the fine and get the extension? Or do you have to pay the fine and do the TM30 right there? What if you don't have the documents from the landord? And what exact document from the landlord are even needed? copy id card (signed), copy lease?

It will auto fill when you complete your address (Province, District and Sub District fields from memory).

The TM.30 has to be done in addition to paying the fine.

If you are renting the place where you are staying, the Bangkok immigration office at Chaeng Watthana is satisfied with a copy of the rental agreement, without any other documents from the owner, ie no copy of the house registration book and ID card needed.
