30 day extension

Hi.  I'm a Canadian staying not far from the Jomtien Immigration Office. I would like to stay longer than my 30 day visa free entry and I do know when I go in and apply, that it will cost 1900b.  What I am wondering, to save time, is what else I would need other than my passport/ticket?  Is there a form I can download before and are photos needed or must I do them there?   Thank you in advance.

Since your going for extension you may wish to think about TM30.

What is Jomtien rules on that for visa exempt extension?

A member familiar will advise.

I deal with CW, bkk and that currently would be a problem.
