A few months ago I got a re-entry permit on my marriage visa thinking we would holiday. We didn't go. Is it still good, or only good for the dates specified?

It is valid until your extension finishes.

It’s good until the expiry date of the Re-entry Permit. It doesn’t matter what dates you wrote on the application form.

I am confused... it is good until my next visa extension? That isn't until December since is marriage visa.

Yes. The expiry date of of your re-entry permit should the same as the expiry date of you current extension of stay ("marriage visa").

Awesome. Do I need to fill out any forms when I get back other than the standard slip of paper they give on the plane?

By law, someone should complete a TM.30 at the address you stay at when you arrive. Whether or not it needs to be done will depend on your local office.
