Is the TM30 needed or not to make a tourist visa extension in Bangkok? 

If it is needed, can we fill it by ourself without any document by the owner at CW?

Probably, but the enforcement is still inconsistent.

No.

If the owner of the place where you are residing is unable or unwilling to file a TM30 of your staying there, you could consider checking into a hotel prior to the day you will be visiting the Immigration Office for your extension of stay. To be on the safe side you could stay 2 days so that you are not 'checked out' already by the Hotel on the moment of your application.
