There is no special forms only for retirement. This TM7 above is the latest version? In another thread someone claimed a slightly different version was the most recent version which he had used at Jomtien. The one he linked looked the same, as far as I could tell, but in the lower right corner of the second page it had a date ... No photo copies of the bank book, and letter from the bank, or are they happy they already have it from when you get the NON "O" retirement extension stamp? You will need copies of almost everything again. Your bank book updated showing the 800k baht has been in the bank for 2 months on the date you happy and another letter from the bank.
