Process to Cancel Work Permit, Change Reason for Extension from Employment to Retirement

For working foreigner, dependent foreign spouse. Appreciate any input on completeness of process below, along with responses to questions underlined.

Step 1. Cancel Work Permit Visa/Extension of Stay

·       -Letter of termination of services from company

·       --Cancel Work permit form (is there a specific form number for this?)

·       -Cancel current extensions of stay (is there a specific form number for this?)

-Are any other forms needed such as tax forms, etc.?

Step 2. Request new 12 month Extension of Stay based on  Retirement for Expat with dependent spouse (assume 800,000 THB deposit)

·      - Must be same day as Step 1

·       -Copy of Letter of Termination from company and cancelled WP

·      - Copies of passport, other original documents

·      - TM-7 forms requesting extension

·      - Bank letter/Bank Book  showing minimum 800,000 THB balance for preceeding 60 days

 

Assuming WP and previous extensions at One Stop, Chamchuri, can Steps 1 and 2 above both be done at Chamchuri or does Step 1 get done at Chamchuri and Step 2 at Chang Wattana on same day?

Will this be a brand new 12 month extension starting from the day of the new extension

Since it was a longish post, all my questions might be hard to see, so I have put them down below, in case anyone knows the answers.

-Does 2 step process have to take place on same day?

When you cancel your Work Permit at the Labour Dept., you will be handed a receipt (Taw Daw 10). That is your proof that you have cancelled it; take it to Immigration with you.  The application form for your new Extension will be given to you at the Immigration office. If you want to avoid an overstay fine,you need to do it all in one day.  Be at the Labour dept. office when it opens at 8:30 a.m.  cancellation takes only a few minutes.  Have a letter from your employer with you stating your day of work.

Hello is some sort of tax form such as PND 1(proof of income tax withheld) required when cancelling work permit or getting the retirement extension.
