Applying for a Special Tourist Visa: How to handle the case where the airlines (United Airlines) is not in the list of airlines in the online application

I plan to fly to Bangkok via United Airlines, which has some authorized to Bangkok. However, when applying for a Special Tourist Visa via the online form,  United Airlines is not  in the list of airlines that I can indicate. What shall I do?

A list of the allowed semi commercial flights into Bangkok is here. Number 49 shows a United codeshare flight from Narita on ANA (see number 41)
