I assume you're talking about for the free Thai public program currently offering only Sinovac and Astra. Correct me if I'm wrong about that.

 

Passports - YES

 

Hi risk/over 60 for June - YES

 

Proof of address - none asked for but I sent a copy of my Thai DL anyway

 

Proof of residence - none asked for

 

Already on record at hospital - not required, you specify the hospital where you want to be vaccinated.

 

Vaccine type - nothing mentioned at this stage.

 

This is an insurance policy for me, if a private hospital offers me Pfizer before the current bunch offer me their shot, I'll go with Pfizer.

 

Also:

An email message to me this morning from the cmcovid team:
 
"We are filling out your registration form and submitting it.
 
Please wait for our contact to book your registration date after the system accepts your registration".
