can i put my name initials on "Name" and full name  in "Applicants signature" or is it the other way around?

Anyway, both fields should include my name right? would be a bummer if one of the fields is reserved for the official who gets my application.

Sometimes the most simple aspects of VISA preparation can be confusing. especially when running out of time.

Just to clarify....what's this declaration for.

I notice it's a bank.

Some docs require to be left undated until filing.

I doubt very much this is one of those. 

The undated ones apply to things such as property transfer etc.

Edit: I just notice purpose is visa. Date it
