That is a rare condition (google claims  notwitshstanding) and I don't know anyone in Thailand with much experience in treating it.

 

The nerve block per se can be done by an anesthesiologist but would have to be ordered by a doctor who concurs with the differential diagnosis.

Thanks @Sherylfor the pointer! 

 

My neurologist suggested the differential diagnostic. He wanted to refer me to a "pain specialist" which can do the procedure. 

I like to get some research done beforehand as the referred doctor is sometimes hit and miss and afterwards I find out that there might be a better option.
