I don't see it. What you have to do though is getting your address registered with immigration and that makes sense - you need to be registered in order for them to to certify your address. That happens via TM30 and possibly TM28 (vs TM47 which is the 90 day report). CW will insist on you having done a 90 day report at that office in order to apply for a residence certificate. This is not the case at most other immigration offices. Alternative is to obtain letter from your embassy. (if they provide that service) That will be accepted for opening bank account and TDL. The posts of a simple form required at CW are correct. At same time I read posts with screenshot of completely different form.
