What date is used in the "I arrived in Thailand on" field? The original date I arrived on using the Visa referenced on the line before it, or the last date I arrived back into the country from my last trip?

I've always used the last date I entered Thailand, not the original (for me) 2006 date.

Thanks guys :thumbsup:, I figured as much, but I wasn't sure when they referenced the original Visa on the line before it.

The OP is asking about TM8.

This refers to date of most recent entry to Thailand (past entry) 

I am also about to apply. What is to be written in the field when and where I plan to leave the country? I don't plan but would like to have the option to leave whenever I want.

Doesn't matter. Pick any date and airport. 

Put the last entry stamp date in PP. (Always)
