A few years ago Pattaya International was 11k, they quoted up to 15k, Urologist is good, head of Queen Sirikit Urology

Might be worth making the trip to Bangkok and getting it done at PDA on Sukhumvit soi 12.  Only a 5,000 baht "donation" required.  Very quick and 'scalpel free'.  I had it done there a few years ago.  You do need to make an appointment though.

FYI PDA in Bangkok aka cabbages and condoms can be a total waste of time so check you meet their criteria. I was too good looking, they didn't want to give me a vasectomy
