Hello Everyone,

 

I would be grateful for some pointers on the best doctor(s) to visit in the city of Korat where I live, or BKK if best, with regards to prostate issues. I just completed a medical check up and the PSA score came in at 4.259 which is just above the 4 limit. My prostate is also enlarged. The hospital in Korat advised I consult with a specialist, hence the message.

 

I would also appreciate knowing if there is anywhere in particular members would recommend that is best for colonoscopy? Has to be one of the most unpleasant experiences but I haven't had one for over 20 years and as I am 60 this year it may be wise to face the matter! My wife probably should go too. Don't think she had ever had the procedure and she is 8 years older than me!

 

Any recommendations for both, and/or feedback on experiences would be welcomed. Thank you!

1. For the prostate:

 

- first of all, get a test of the free PSA and (resultant) free-to-total PSA ratio as this is very helpful in distinguishing between elevations caused by Benign Hyperplasia vs malignancy.

 

-then consult one of these specialists:

Same doctor, 4 different hospitals. SIPH while least costly is not so easy to deal with/get appointments promptly. Thonburi costs are just a fraction more and much easier to deal with but, like SIPH, on the Thonburi side of the river.  MedPark and Bumrungrad are centrally located.  Assuming you do not otherwise have anything to do or place to stay on Thonburi side, I would probably opt for MedPark as best tradeoff in terms of cost and location. But you can see him at any of these places, up to you.

 

 

2. For routine screening colonoscopy,  any of the hospitals in Korat (Bkk Hosp, St Mary's, main govt hospital  etc) should be OK or you could also do it at hospital where you consult urologist. Should be able to, yes.

 

The free to total ratio is no help in distinguishing elevations due to prostatitis vs cancer but is  helpful in distinguishing between elevations due to BPH (benign enlargement) vs cancer.

 

Could help inform the decision of whether or not to proceed to additional tests such as MP-MRI.

 

Results of prior PSAs also useful for comparison.
