I am going through the painful process of getting my yellow house book and pink id card from the local amphur. Just when we thought we had everything in order and we were ready to go, we have discovered that my US passport and the required translated page must be authenticated by the US Embassy before the translated passport page is submitted to the Ministry of Foreign Affairs to be stamped and then taken to my local amphur. The MFA approval is require by our local amphur. I have never heard of this requirement especially after talking to many expats who already have yellow books. This means a five-hour drive to Bangkok, hotel, embassy fee, trip to MFA (do I need an appointment I wonder} then the five-drive back home. I tried to persuade the local munchkin at the amphur to drop the MFA stamp requirement for the translated passport page but she wasn't having any of it.
