The official MFA application form for visas (which should be accepted by all Consulates) is

This is interesting now, I remember Lopburi3 posting that info about the form before

but I've taken a closer look at it. On the Savannakhet consulate form, despite having

of course the different address in the heading, there is also a special line for the bank

note serial #'s. And they want it filled out. That is where the forms are different and

why I'm still not sure if you can use any form but the one from the specific consulate.

Did we ever get any confirmation on successful "field use" of the MFA form?

I would always enter an Immigration Office with a plan and subsequently with available forms previously filled-in.

If they do not like it, I will use their forms. Why bother?

Generally a good idea to prepare ahead of time. With these visa application forms they often specify

that the photos are to be glue-sticked not stapled and the queue is often such a jostling mob leading

to a narrow entranceway of a small application processing room, it can be kind of a bummer not to

have the correct form and often leave you in a serious time constraint for a possible 2nd attempt.
