you'll have to go out of the country, if you want a single entry , the only option available if you don't have a work permit ,almost any consulate will do the job

The health certificate is possibly the easiest part of it...just go to the clinic on the corner and they will issue one. Costs around 100 Baht and chances are you will not even see the doctor!

Once the WP is issued, you take it to the immigration with another truck load of paperwork and if you qualify you can get the one year non-immi B.
