I believe people have mentioned a tic place for more than one entry and do not see that on the above (although very small and my eyes are old). Don't see in jumbo size either so expect it is the old form.

Yes, that is the new form .

However I am not sure if they will accept the form as the Original form is also printed on the back.

That's the new form but it prints a little bit smaller than the original. Try centering and printing at 118 percent. The printing on the back is the same as on the old form.

They will not accept the old form.
