Pinched nerves are usually associated with some numbness or pins and needles in the arm or area supplied by the nerve in question. Better to have a neck X-ray in these cases to confirm a diganosis before attempting manipulation.

Prolonged episode of pain usually due to muscle spasm and several physio or massage treatments will be necessary. Massage can work but only if the therapist knows what he/she is doing.

The "pain in the neck" feeling after waking up in the morning is usuall caused by a position of the head when sleeping that allows for the cervical paravertebral joints to open (head at an angle) and, when the head is rotated back to the normal position, the membrane around the paravertebral joint can get caught between the bony surfaces and pinched. The membrane is quite sensitive and this will cause immediate neck muscle spasm across that joint.

If done immediately after onset of pain, gentle manipulation may reverse the cause but usually, by the time you wake up, the muscle spasm is set, keeping the membrane pinched and the whole vicious circle starts.

Gentle traction, massage and very gentle manipulation/mobilisation usually helps.

Once the lower neck/upper shoulder muscles become involved, the process is much more difficult to manage.

Short course of anti-inflammatory meds plus muscle relaxants can help if given early.

The key is to see someone qualified like a chiropracter, osteopaths etc. A good practioner will do a thorough evaluation including detailed questionaire of symptoms, X Ray, range of motion, clinical examination of veterbra, posture analysis, muscle imbalances etc.

Trying to diagnose serious problems on the internet or with limited information is foolish. The OP could have any host of issues ranging from dehydration to a spinal tumor.

Do your self a favor. Tell the forum where you live and ask for some recomendations of professionals to seek out.

I have had some of best work ever from a guy here in Chiang Mai who does Shoalin Tendon massage. He adjusts joints and vertebra like a chiropracter, deep tissue like a rolfer and he manipulates tendons and nerves directly. It can be fairly painful at times but I would put him up against anyone I have ever tried and I have been a bodyworker for 10 years.

I would like to hear from others about who they would seek out for therapy with these types of symptoms.

For 20 years, I had problems with my neck, shoulder, and back. I went to several chiropractors, and the pain would lessen for a while, but then return. I finally went to a rehabilitation clinic, and they solved my problems almost immediately. They had a hard foam cylinder, about 8" in diameter and 4 feet long. While on my back, I placed it under my neck and, pushing with my feet, rolled until the foam was at my waist. I did this a couple dozen times and my vertebrae, bones, tendons, and nerves aligned - it sounded like fireworks - crack crack crack crack. I use it once a week, even though the pain has not returned for 6 years.

I asked them about buying a roller and was told, since it was a piece of "medical equipment", it would be $300. The nurse then told me to "go to Bob's House of Foam" (yes, there was a company by that name). Bob sold me the same thing for $2/foot.
