Hello, question 6.3 on the vaf4 asks if you have been refused a visa before. It asks for dates and reference number. Unfortunately the refusal paperwork has been lost, what should i do? Thanks for any help.

Its in the back of her passport the stamp for refusal.

Obviously you must answer 'yes' to this question, and give as much detail as you can, explaining that you no longer have the paperwork.
