Now Air Asia is offering direct flights from to Phuket to new international destinations, including Medan, starting in December (but you can book now). So is it possible to get a new visa in Medan?

Surprise to me, but there is a Consulate there, most likely an "honorary" one. Best to email or phone to be sure.
