"letter Of Consent" In Thai. Any Links?

I have found a template for a "Letter of Consent" in English.

But I haven't been able to find a copy in Thai (which I need as well).

The "Letter of Consent" is a statement from person "a" in which he/she allows person "b" to travel abroad with their child.

Has anybody used such a letter before, and was the letter accepted without hassles at the airports?

PS: if anyone can translate the "Letter of Consent" attached, this will be fine.

It's for traveling abroad.

That's why I have the English version already.

But I have been told by people who have used the "Letter of Consent" in English only, that they had troubles at leaving Thailand.

So, better be prepared and have it as well in Thai.
