I trust Khota Bharu and Penang are still not issuing double entry.

Only Vientiane will issue double entry tourist visas for free in the region. The official rule is that only single entries are free.

Phnom Penh has been issuing double entry tourist visas too. I know one lady who has had 2 from there this year. She went through an agent.
