Certificate Of Origin/Form A

Does anyone know who can issue these in Chiang Mai. I need to send things by DHL but they don't do these and don't have a clue; although I have got from UPS in the past no problem.

Other than switching to UPS, does anyone know of a shipping company/etc that can issue just the form.

- Product must be comply with the rules of origin specified by each GSP schemes

- a commercial invoice

- a bill of lading or an air waybill

- a paper of cost production declaration and/or a paper of production process declaration for textile products

- a copy of Form A applicant registration certificate.

DHL in CM got me one a few months ago, if agent is OSPT EXPRESS SERVICE, 47710 SAMLAN RD T PRASING A MUANG... Otherwise Dept. of Foreign Trade is out by the airport I believe...
