Extending Non-O On The Last Day

Hi, I would like to extend a one year non-o visa based on retirement. Current Visa expires on May 3rd. I understand all the requirements except the criminal history record. Been a few years since I did a retirement visa and not familiar with this doc. Just got in the country on May 1st and do not want to waste a trip to Mai Sai on Tuesday if i cannot provide all documents as visa expires on that day. Is this form required or is it discretionary? Will be going with my Thai wife and son for the family effect.

If you are applying for a 12 month extension based on retirement a Medical Certificate or a Police Report is not needed.

You donot need the wife or son for effect either if you meet all requirements.

You appear to say that you last arrived in Thailand on May 1st with a multiple-entry non-O visa, and this visa expires on May 3rd. Therefore, on your arrival on May 1st you should have received permission to stay for 90 days, ie until July 29th. Look at the arrival stamp in your passport, which should be something like this:

Are these the dates shown on your latest arrival stamp?

Perhaps it's an old extension or O/A entry to the 3rd May 2011 and he arrived on May 1st with a re-entry permit.

Or maybe if May 3rd is the visa enter/use before date, then he has a 90 day stamp as Maestro suggests, or even a one year stamp if he has an O/A Visa.
