Filling Out A Tm. 8 (Re-Entry)

On the back page where it says "My previous visa for Thailand", do I put my last extension of stay info. or non-immigrant ( there is no info. I have a new passport and it just shows that it has been transferred ). Also, where it says where "I arrived", do I put the original date (prior to the extension of stay), or the last entry into Thailand?

" I arrived in Thailand on " question is your last entry date into Thailand.
