I need to apply for for a teachers license.

The application form on the web site looks to be really old (it's talking about applications before 2003)

Any one have any up to date info about what documents are needed?

Also if i was to apply for a waiver, what docs are needed?

If you do not have a TL already you will need to apply for a 'waiver'.

What qualifications do you have and have you completed the Thai culture course? (now suspended of course)

The school should be making the application for the TL/Waiver on your behalf.

Getting the actual license may take some time and so a waiver is usually granted while the paperwork is in process.
