I am unsure how to fill out the US passport nenewal form DS-82.

No. 8 on form they want my mailing address, I have a P.O. box in the US, but no permanent address there as I have been in Thailand for a year. Do I give my mailing address in Thailand or my P.O. Box in the US?

No. 7,They want my primary Phone No., do I give my Thai No.?, I have no No. in US

No. 18 They want my permanent address, do I give my Thai address, I have no other., the form is not set up for an out of country address.

You submit the form to the US Embassy here rather than mail it with passport (which it is designed for) and fill out on-line and then print it out. You will be using Thai address/phone numbers as this is where you live (form is designed for submission by mail with passport of those in USA but here you give to Embassy and keep passport).

Correct. You simply use your Thai address and phone number(s) for the form. I've done it twice now, once for my primary passport and once for my two year secondary passport. No hassles, the folks at the ACS counter are very helpful.
